Prior to making an appointment with us, please check our list of contracted insurance plans. While we are contracted with most types of insurance, we are also available for consultation for those with non-contracted plans.

Please print our patient registration forms and fill out all four pages prior to your appointment.

  Any pertinent records from other doctor visits (fax or bring to office)
• Tests
  The results of any specialized tests: Hearing and balance tests, sleep studies, etc.
• X-rays
  Bring any x-ray reports and actual films of CT and MRI scans
• Medications
  Updated list of current medications
• Current Insurance Card
• Interpreter
  If appropriate, bring an English-speaking family member or friend
